SMaRT Med (SMaRT Health & Wellness) is eager to provide orthopedic sports medicine, hand surgery, and physical and hand therapy services to our community. We believe it’s critical to communicate our participation with insurance payers to you. Please read this carefully, as there are two business entities involved providing you care. There will be updates over the next few months as we continue to work with the insurance companies.
SMaRT Med provides care as a group practice and is in the process of establishing new insurance payer contract agreements.
Dr. Ryan Labovitch provides care under his individual private practice, and will continue caring for his patients under his current contract agreements until further notice.

Patients receiving care from Dr. Raymond Raven, or Dr. John Kwon and the rest of the Physical and Hand Therapy team, please read the following carefully.
As of November 1, 2023, SMaRT Med participates with the following insurance companies.
- Medicare (Part B)
- Anthem Blue Cross (Commercial PPO)
- Blue Shield of California (Commercial PPO/EPO Standard Network)
- Cigna (Commercial PPO)
- United Healthcare (Commercial PPO)
- Aetna (Commercial PPO, Managed Choice, EPO, and Medicare PPO)
- HealthNet (Commercial PPO, EPO, POS, and Medicare Advantage PPO)
*NOTE: HMO plans for any insurance company will NOT be accepted at SMaRT Med now or in the near future. Medicare Advantage plans are accepted for all insurance payers we are contracted with. Even after becoming contracted with an insurance payer, the providers need to become credentialed with the insurance payer network. Please note this may delay scheduling services until the provider is fully credentialed.
Our offices will ensure your continuity of care should there be any change in the provider you see, and we will provide medical records as requested and legally obligated in the appropriate scenarios. Please contact our scheduling team if you need more information or clarification.
